Have worked out how to use it with 4E but not really used it much so far:
You need to put monsters in Stock portfolios rather than normal ones. This can be done by just renaming a portfolio with the .stock extension and moving it to the system data folder (rather than the user data folder where portfolios are normally saved). Any monsters in stock portfolios will then show up in the encounter builder to be selected. If you have any success, post here as I'd be intrigued about how well it works for 4E.
